In the near future, a totalitarian society is plagued by a dangerous drug called Substance D. Undercover detective Bob Arctor is assigned to a small group of drug users, aiming to infiltrate the drug's network. As he delves deeper into this dangerous underworld, his own identity starts to unravel, and he experiences schizophrenic behavior.

Bob's investigation is supported by the New Path Corporation, a recovery center that appears to have ulterior motives. As Bob's mental state deteriorates, he is subjected to tests to assess his sanity. As the line between reality and illusion blurs, it becomes increasingly difficult for Bob to discern who he can trust.
