Middle-aged, sexually repressed Sylvia Stickles is the subject of this John Water's film, set in North Baltimore. A Dirty Shame (2004) is a John Waters film set in North Baltimore that centers around Sylvia Stickles, a middle-aged woman with a strong aversion to sex. Sylvia refuses to have intimate relations with her husband Vaughn Stickles, and she keeps her well-endowed daughter, Caprice, locked in her room. Sylvia is serving home detention for moral depravity charges and is determined to promote decency on Harford Road with her mother, Big Ethel. They lead a group known as the neuters.

However, Sylvia's life takes a drastic turn when she is accidentally hit on the head by a lawnmower hanging out of a passing pick-up truck. The blow to her head completely changes her sexual behavior from being a prude to becoming a prostitute. This unexpected transformation leads Sylvia to meet a secret underground sexual community known as The Others. She becomes their leader and encourages them to embrace their desires and indulge in unconventional sexual acts.

As Sylvia's newfound sexual liberation causes a public uproar, the prude community is determined to put an end to her influence. They form an opposing group called The Missionaries and engage in a heated battle against Sylvia and The Others.
